Personalised Coloured Questionnaires for that Professional Touch to Impress your Customers

You can have A5 questionnaires which are of high quality print, with your name, address and logo, if desired. This A5 format makes them less intimidating, so your customers can see they are easy to fill in. Experience of summarising many thousand returned questionnaires has resulted in a redesign of our form for this year, although the same questions must be asked. The front page incorporates the required instructions to the patient. The back page offers our address as an alternative return point, again as required by the government. You will get a 40% excess to cover any spoilt or lost forms and one copy of the large and one of the extra large format which you can photocopy if the need arises (although it rarely does).

The package also includes a comprehensive SOP which, if followed, means you should complete the survey within two weeks, a label to put on your ballot box (any cardboard box with a slot cut in it will do) so that patients can anonymously return the forms and a postage paid addressed jiffy bag for your returns to us. You can always pay extra if you want recorded delivery.

 

Summarising your Survey results

When you return the completed questionnaires we will send you a certificate telling you  the date we received it, as proof for your PCT that you have completed your survey. Under the contract you only have to show your PCT your action plan.  Then you don’t have to do anything but await the completed Reports in  the post.

It will take around two to three weeks for us to enter the data into our computers, analyse, and write your report but please note, the later you send your surveys to us the longer it will take us to get yours done due to increased work loads

 

Your Results will contain

1) A coloured graphical summary of your returns

2) An analysis of the graphs by one of our directors (each drawing on 20 years of  community pharmacy experience).

3) An action plan form which you can complete and show to your PCT as required under the clinical governance section of the Contract

4) A colour poster for display in your pharmacy showing your best customer comments and results of the waiting time, the staff and the overall verdict.

5) Around 3 months after the year end,  you will see on our website a further report detailing all the pharmacies we have summarised, so you can compare some of your numbers with the other pharmacists we have summarised.

6) If you have over 10 branches you will be sent coloured graphs showing your shop’s overall rankings against each other
